Medication Consent Forms
Medication Consent Forms are required for all medications administered at school.
"Medications" also include any over the counter medications. Per Texas Board of Nursing, the school requires a doctor's order to administer these.
Medication and care forms do not carry over from one year to the next. Each year, a new form must be submitted to the nursing team.
Please download, complete and bring the appropriate form with any medication your student will need for school administration.
Seizure Action Plan and Diabetic Care Plan: your physician will generate this for you.
